Aircraft Charter Agreement

This agreement is between Lyon Aviation, Inc., located at 832 Tamarack Road, Pittsfield, MA, 01201, Phone: (800) 816-7625, Fax: (413) 499-4335, and the Client identified below for the purpose of arranging aircraft charter services as specified below.

Total Price		$. This price is inclusive of all landing fees, complimentary snacks & beverages, international fees and pilot expenses. Special catering or flight phone usage is NOT included and will be charged after flight is complete. Additional expenses may apply due to unforeseen weather conditions or changes in flight itinerary. These expenses may include but are not limited to hangar fees, deicing, and if rerouted due to weather conditions any additional flight time. is responsible for the collection and payment of any and all international, federal, state, or local taxes.

Payment Schedule		Payment must be received by Lyon Aviation, in the form of a wire transfer, certified bank check, or money order within 72 hours of the scheduled departure.
If the aircraft is unable to complete any portion of the itinerary due to adverse weather, operational, or mechanical reasons, Client shall be notified immediately and Lyon Aviation shall refund to Client all monies paid with respect to the portion of the cancelled charter flight legs, to the extent paid by client.
Cancellation Terms		25% of the total charter price will be assessed to the Client if cancellation is made less than 72 hours prior to departure date listed above.
If the aircraft is cancelled less than 72 hours prior to departure time listed above, Lyon Aviation shall refund to Charterer all monies paid with respect to the cancelled charter flight less 25% cancellation fee.

To confirm the above referenced charter flight please print this page and fill it out. Make sure to initial the itinerary and sign the acceptance. Then fax the entire agreement to Lyon Aviation at 413-499-4335.
